Driver License Policy

Here at Premier VetCare protection of your personal information and account is of up most importance to us.  As we are often extending a temporary credit account by providing services prior to payment we need to follow some credit and information laws.  Also in order to prescribe controlled medications for your pets we are required by law to report those medications to the State of Tennessee on a weekly basis.  In order for us to be in compliance with a multitude of state, and federal laws that apply to veterinary medicine and businesses we require that a photo ID, and the state Drivers License is the one form that has all pertinent information we need, be on file with our practice.  By having the photo ID on file we will not need to ask to review your ID at each visit.  This would slow down our check in and check out processes.  Without a Drivers License on file we will not have the information we need to verify your identity and for proper legal record keeping and reporting.  Below is a list of just a few reasons we must have that information of file.

Protect your info from software breach.

Though we do have many software and hardware protections in place, database breaches remains a top concern for business.  A photo copy allows us to have the needed information on file with out entering it into our practice database.  This protects your information from being accessed in the event a cyber-breach happens to our software.  Being an image held in a separate file adds an extra layer of protection as images are not the target of database breaches.

Meets our legal reporting requirements.

We must confirm your home address, and birth date for reporting controlled drugs to the State of Tennessee given to you on behalf of your pet.  These laws are in place to track control substance abuse and drug seeking clients. 

Confirm you legal address. 

The Tennessee practice act requires us to place your legal address on all prescriptions.  So to ensure the address we have been given is accurate we confirm that information with your legal drivers license.

Protects your account from fraud and identity theft.

Having a photo ID on hand allows us to confirm that it is you that is using the account.  If suspicion is raised that an unauthorized person is using the account we will be able to confirm if a user is you or not, and protect your account form identity theft and fraud.  Unfortunately we have had clients try to pose as a family member, friend, or ex-partner, to run up a bill and try not to pay it.  This places the account holder liable for those services if we can not identify who actually used the account.  Though we do get to learn the faces and names of many of our clients, its impossible for us to remember the faces of the 20-30 thousand clients we server.

Your co-operation and understanding is appreciated.  If we truly did not need this information on file we would not collect it, as we too are as concerned about protecting your private information as you are. To date we find this is the best way for us to protect your account from fraud and collect and confirm the required information of us to meet the legal requirements of the State of Tennessee, as well as Federal regulations.  Without the main account holders Drivers license on file we are not able to set up an account or provide services.

Life Threatening Emergencies, Appointments, Urgent Care Walk-ins, Wellness Walk-ins

When bringing your pets into our office, for their and the safety of other pets, please either have your pet leashed or in a pet carrier. Please ask our staff at check in for a leash if needed.
